Each state Medicaid program maintains a Preferred Drug List (PDL) or formulary. This list specifies which medications are covered without prior authorization. Medications on the preferred list are selected for their therapeutic efficacy and cost-effectiveness.

If a doctor prescribes a non-preferred drug, the pharmacy will require prior authorization. This process requires the prescribing provider to document that the preferred alternatives were ineffective or caused adverse reactions before the state will approve coverage for the non-preferred drug.

Copayment Limits and Protections

Federal rules limit the amount of cost-sharing that states can impose on Medicaid beneficiaries. For prescription drugs, copays are typically nominal, often ranging from $1.00 to $4.00 for generic medications and up to $8.00 for brand-name drugs.

Crucially, federal law prohibits providers or pharmacies from denying services or prescriptions to individuals who cannot afford the copayment at the time of service. While the beneficiary still technically owes the debt to the pharmacy, their access to vital medications is legally protected.
